Our model delivers personalized care through telehealth, home visits, and care coordination services—spanning Primary Care, Chronic Care, Women’s Health, Sexual Health, Mental Health, and Medication Management.

As we expand our chronic care and pharmacy support services, we are seeking a Clinical Pharmacist to partner with Nurse Practitioners and support patients who require complex medication management.

Position Overview: 

This is a contract-based, per diem role to start—ideal for a PharmD-level Clinical Pharmacist who thrives in a remote care environment. You’ll collaborate with our care team to deliver virtual pharmacotherapy consultations, optimize medication adherence, and support chronic condition management for patients—many of whom are uninsured or medically underserved.

This role is fully remote with flexible scheduling, offering a unique opportunity to contribute to holistic care delivery across multiple disciplines.

Core Responsibilities:

• Conduct virtual pharmacotherapy reviews and consultations for patients with chronic conditions (e.g., diabetes, hypertension, asthma).
• Partner with NPs and clinical staff to optimize medication regimens, minimize risk, and improve outcomes.
• Perform medication reconciliation for new patients, care transitions, and polypharmacy cases.
• Assess and address adherence barriers, working directly with patients to improve medication usage.
• Evaluate medication safety, dosing, contraindications, duplications, and interactions.
• Provide evidence-based recommendations to prescribers for ongoing therapy management.
• Document clinical consults and care plans within EMR or internal care tracking systems.
• Participate in virtual care huddles, team discussions, and improvement projects.
• Support care transitions, referrals, and follow-ups in collaboration with other DiscoverU Health specialists (e.g., Dietitians, Behavioral Health, Primary Care).

Qualifications:

• Doctor of Pharmacy (Pharm.D.) from an ACPE-accredited college or university.
• Minimum of 1 year of experience in a clinical pharmacist role.
• Completion of a clinical pharmacy residency or equivalent relevant experience in chronic care or ambulatory care.
• Active, unrestricted pharmacist license in Maryland, D.C., or Virginia (multi-state preferred).
• Strong knowledge of chronic disease states, medication counseling, and MTM (Medication Therapy Management).
• Experience with EMR systems and remote patient interaction is a plus.
• Excellent communication, attention to detail, and patient-centered care skills.
• Authorized to work in the U.S.
